Indywidualny implant tytanowy gałęzi żuchwy – problemy diagnostyczne i lecznicze
Title: Custom-made titanium mandible implant – diagnostic and treatment challenges
Streszczenie: Pacjentka zgłosiła się do poradni przyklinicznej z powodu guza gałęzi żuchwy, który deformował okolicę przygardła, klinicznie i radiologicznie sugerującego torbiel lub szkliwiaka. Na podstawie biopsji aspiracyjnej cienkoigłowej (guz mieszany), ponownego badania histopatologicznego (fibromatoza), biopsji eksploracyjnej w trakcie usuwania trzeciego zęba trzonowego z tej okolicy (torbiel) oraz badania śródoperacyjnego (tkanka mezenchymalna zbudowana z komórek wrzecionowatych z ogniskowym szkliwieniem, bez martwicy) otrzymywano różne rozpoznania, inne od rozpoznania ostatecznego (fibromatoza). Po usunięciu gałęzi żuchwy wraz z wyrostkiem dziobiastym i kłykciowym wprowadzono idealnie dopasowany do warunków anatomicznych żuchwy tytanowy implant. W związku z ostatecznym rozpoznaniem i podejrzeniem w TK pozabiegowym nacieczenia struktur podstawy czaszki (artefakty udzielone z implantu tytanowego) byliśmy zmuszeni do usunięcia implantu w celu poszerzenia zabiegu (3 miesiące od pierwotnego) i ponownego wprowadzenia implantu po upływie roku.

Summary: A patient was referred to our Outpatient Department because of the mandible tumour that buckled the parapharyngeal area. It had the clinical and radiological appearance of ameloblastoma. On the basis of the fine-needle aspiration (pleomorphic adenoma), recalled incisional biopsy (fibromatosis), exploratory biopsy during the removal of the third molar in this area (odontogenic cyst) and intraoperative biopsy (mesenchymal tissue consisting of spindle cells with focal glassy degeneration, without necrosis), a variety of diagnoses was obtained, differing from the final diagnosis (fibromatosis). After the surgical resection of the mandible ramus with the coronoid process and the condyle, the perfectly anatomically fitting titanium implant was introduced.
