Anaphylactic shock

Anaphylactic shock is an acute allergic reaction with a rapid onset and course. To diagnose anaphylactic shock, symptoms must involve at least two organ systems: the upper airway (swelling), the respiratory system (wheezing or cough), the cardiovascular system (hypotension), and/or the skin (swelling, rash). Untreated anaphylaxis may lead to cardiac arrest; therefore, treatment must not be delayed, and epinephrine should be administered into the anterolateral thigh at an age‑appropriate dose before emergency medical services arrive.​

Adults: 0.5 mg (or an adult EpiPen).​ Children: 10 micrograms/kg body weight (or a paediatric EpiPen).​ If there is no improvement within five minutes, administration of another dose of epinephrine at the same dose should be considered.​

Hypoglycaemia​

Hypoglycaemia (< 70 mg%) may occur in patients with diabetes treated with insulin or oral hypoglycaemic agents who have not consumed an adequate amount of food due to their dental appointment. The first symptoms (at blood glucose < 54 mg%) include agitation, hand tremor, and pale, markedly diaphoretic skin. If left untreated, hypoglycaemia may lead to impaired consciousness, including coma.​

Management: provide simple carbohydrates orally, for example half a glass of fruit juice. After 15 minutes, reassess blood glucose; if it is unchanged or lower, administer simple carbohydrates again. If the patient’s condition improves, they should then consume [...]
