Application of rotary files in everyday endodontic practice

The principal determinant of the success of endodontic therapy is mechanical preparation of root canals with their thorough disinfection [1]. In their research Kakehashi et al. confirmed that the presence of microorganisms in the pulp tissue can trigger infection and periapical changes. This is why elimination of the greatest possible number of microorganisms through chemical and mechanical preparation of the canals is the foundation of every endodontic therapy [2].

Conventionally, root canals were extended with manual instruments. Such instruments were made of stainless steel which made preparation much more difficult. The progress in dental materials science has led to development of nickel titanium alloys that revolutionised the endodontic market. The nickel titanium (NiTi) instruments are made of an alloy composed of 55% nickel and 45% titanium. This composition ensures that the material is flexible and has a five times lower flexibility module compared to instruments made of steel. The specific shape of crystal structure allows for reversible change of the shape of an instrument, which gives NiTi instruments shape memory that enables preparation of curved root canals. The instruments are not as hard (HV-320) as steel instruments (HV-520). The hardness of dentin is HV-40. NiTi instruments are manufactured in a traditional way, by milling semi-fabricated products [3].
